Applications accepted	All enrollment forms must be completed in full before your child begins attending holy trinity. the medical form must be signed and dated by your child's physician and must be updated annually through 6 years of age. all of these forms will be provided by the administrator at the time of your visit to our center. a tour of the facility may be arranged by contacting us at 419-382-4088.
